About the Opportunity As a Social Worker you will provide quality trauma informed, person centred and culturally responsive clinical Social Work services to support patients/families/carers in their emotional response to the impact of illness and/or to facilitate transfer of care from hospital to community based services to support optimal health outcomes. In this role you will: Providing quality Social Work evidence based practice within an environment of competing complexity, demands and priorities. Maintaining a high volume of diverse and complex casework and service demand. Provide Social Work clinical practices including psychosocial assessment, crisis and brief intervention, case planning, liaison, counselling, advocacy, and service referral. About You We are looking for someone who has: Tertiary qualifications in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW). Knowledge and understanding of the psychosocial issues that arise for patients and their families/carers when experiencing chronic/acute illness and trauma. Demonstrated experience, preferably within a clinical service setting in providing psychosocial assessment and Social Work responses such as crisis and brief intervention, case planning/coordination, liaison, counselling, advocacy, and service referral.
